Acceptance

By creating an account you agree to these terms. If you do not agree, do not use the app.

Not a medical service

Cognitive Wellness is informational and supportive only. It is not a medical device, not a substitute for professional diagnosis, and not a substitute for treatment. The self-screening tool is reflective only, not diagnostic. In an emergency call your local emergency number.

Your account

You are responsible for the accuracy of the information you enter (medications, contacts, etc.) and for keeping your login credentials private.

Subscription & trial

The app offers a free trial. After the trial, continued use requires a subscription. Subscriptions auto-renew until cancelled. You can cancel any time from your account settings.

Acceptable use

Do not use the app to harm others, upload illegal content, or attempt to access another user's data.

Termination

You can delete your account at any time from Settings. We may suspend accounts that violate these terms.

Disclaimer

The app is provided "as is" without warranty of any kind. To the maximum extent permitted by law we disclaim all liability for indirect or consequential damages.
